MTC CANCELLATION / NO SHOW POLICY
We, at Medical Transformation Center, value your time and look forward to caring for you and your family on this journey to health transformation. Our providers and staff make every effort to be punctual and give advance notice of any alteration in the schedule. We appreciate and expect the same courtesy from our patients.
New Patients (Provider Appointments):
Cancellations:
Prior to the 2 business day cancellation period:
- If you must cancel (and do not reschedule) your Initial Consult appointment and do so prior to the 2 business day cancellation period, you will receive a full refund of your deposit.
- If you must cancel (and do not reschedule) your First Follow-Up appointment and do so prior to the 2 business day cancellation period, you will receive a $400 refund ($400 refund of the $1200 paid).
- If you must cancel (and reschedule) your Initial Consult and/or First Follow-Up appointment and do so prior to the 2 business day cancellation period, no fees will apply.
After the 2 business day cancellation period:
- If you must cancel (and do not reschedule) your Initial Consult appointment and have not done so prior to the 2 business day cancellation period, you will receive a $200 refund ($200 refund of the $400 deposit).
- If you must cancel (and do not reschedule) your First Follow-Up appointment and have not done so prior to the 2 business day cancellation period, you will not receive any refund (no refund of the $1200 paid).
- If you must cancel (and reschedule) your Initial Consult appointment and have not done so prior to the 2 business day cancellation period, we reserve the right to charge the card on file a $200 fee for each occurrence ($200 in addition to the $400 deposit).
- If you must cancel (and reschedule) your First Follow-Up appointment and have not done so prior to the 2 business day cancellation period, we reserve the right to charge the card on file a $400 fee for each occurrence ($400 in addition to the $1200 paid).
*After the second cancellation within the 2 business day cancellation period, we reserve the right to require payment up-front for any future appointments.
*Any exceptions are to be reviewed by management and will be evaluated on a case-by-case basis
No Shows*:
*The No Show policy assumes you have not canceled the appointment prior to the 2 business day cancellation period.
- If you do not show up for (and do not reschedule) your Initial Consult appointment, you will forfeit your $400 deposit.
- If you do not show up for (and do not reschedule) your First Follow-Up appointment, you will not receive any refunds (no refund of the $1200 paid).
- If you do not show up for (and reschedule) your Initial Consult appointment, we reserve the right to charge the card on file a $400 fee for each occurrence ($400 in addition to the $400 deposit).
- If you do not show up for (and reschedule) your First Follow-Up appointment, we reserve the right to charge the card on file a $800 fee for each occurrence ($800 in addition to the $1200 paid).
Existing Patients (Provider Follow-Up Appointments):

If you must cancel your appointment and have not done so prior to the 2-business day cancellation period, we reserve the right to charge the card on file according to the percentages listed below.
- 50% of the appointment fee for the 1st occurrence (varies per provider)
- 100% of the appointment fee for the 2nd+ occurrence (varies per provider)
*After the second cancellation within the 2-business day cancellation period, we reserve the right to require payment up-front for any future appointments.

No Shows:
If you do not show up for your appointment and have not canceled the appointment prior to the 2-business day cancellation period, we reserve the right to charge the card on file according to the percentages listed below.

IV / Emsella / Aesthetic Appointments:
Cancellations:
If you must cancel your appointment and have not done so prior to the 1-business day cancellation period, we reserve the right to charge the card on file according to the amounts listed below.
- $50 for the 1st occurrence
- $100 for the 2nd+ occurrence
No Shows:
If you do not show up for your appointment and have not canceled the appointment prior to the 1-business day cancellation period, we reserve the right to charge the card on file $100 for each occurrence.
*After the second ‘no show’, we reserve the right to refuse treatment and/or require payment up-front for any future appointments.
*Any exceptions are to be reviewed by management and will be evaluated on a case-by-case basis.
